Knee Cartilage Repair Recovery Supplements

Recovery Guide · Knee Cartilage

Cartilage repair procedures such as MACI, microfracture and osteochondral grafts (OATS) have some of the longest recoveries in sports medicine. Up to six weeks of protected weight-bearing, followed by months of careful loading, put the quadriceps under heavy pressure.

Nutrition can't regrow cartilage on its own, but it can help protect the muscle that shields your repair and supply the building blocks for tissue healing.

Why cartilage repair recovery is so demanding

After microfracture on the femoral condyle, patients commonly stay non-weight-bearing for about 6 weeks, according to a review of rehabilitation after cartilage procedures. After MACI, the typical plan is protected weight-bearing for about 6 weeks, with progression to full weight-bearing by around 12 weeks to protect the graft.

That long period of reduced loading causes significant quadriceps weakness. A review of quadriceps dysfunction after joint preservation surgery notes that strength deficits after cartilage restoration can persist for years. Strong quads matter because they absorb load that would otherwise pass through the repaired cartilage.

Key nutrients for cartilage repair recovery

Essential amino acids with leucineProtect quadriceps muscle during non-weight-bearing. In a randomized knee surgery trial, essential amino acids reduced thigh muscle loss by about a third compared with placebo.
Collagen peptides + vitamin CCartilage, tendons and ligaments are collagen-rich tissues. In a 12-week trial of 139 athletes with activity-related knee pain, 5 g of collagen peptides a day reduced knee pain during activity compared with placebo.
HMBHelps slow muscle breakdown during the weeks you can't load the leg.
Creatine monohydrateSupports strength gains when progressive quad loading begins.
Vitamin D3 + K2Supports bone health under the repair, which matters especially after osteochondral grafts that rely on bone integration.

Typical recovery phases

Weeks 0–6Protected or non-weight-bearing, often with continuous passive motion. Muscle loss is fastest, so protein and amino acid intake matter most.
Weeks 6–12Progression to full weight-bearing and full range of motion.
Months 3–6Return to low-impact activities such as cycling, swimming and golf, as described in MACI's patient guidance.
Months 9–12+Return to higher-impact sport, depending on lesion size, location and your surgeon's protocol.

How the OVIAL system fits

OVIAL PRE builds amino acid, collagen and vitamin D reserves before surgery. OVIAL POST then delivers essential amino acids, whey protein isolate, 15 g of collagen peptides, HMB and creatine daily through the first month of non-weight-bearing, when the quads are most at risk. Given how long cartilage rehab lasts, many patients continue POST through the weight-bearing transition.

Frequently asked questions

Can supplements regrow knee cartilage?
No supplement has been shown to regrow cartilage. The goal of nutrition after cartilage repair is to protect muscle, support connective tissue healing and give the surgical repair the best environment to mature.
Is collagen good after cartilage surgery?
Collagen peptides with vitamin C supply building blocks for collagen-rich tissue, and trials in athletes show reduced activity-related knee pain. Ask your surgeon about timing.
How long is recovery after MACI or microfracture?
Expect about 6 weeks of protected weight-bearing, full weight-bearing by around 8 to 12 weeks, and return to sport after roughly 9 to 12 months or more.
How do I prevent quad atrophy after cartilage repair?
Follow your PT's activation exercises, get enough protein (around 1.2 to 1.5 g per kg of body weight per day unless your doctor says otherwise), and consider essential amino acids, which have trial evidence for reducing thigh muscle loss after knee surgery.
